New Law Requires Vision Exam For All Entering Kindergarteners

Senate Bill 641 was passed and since 1/1/08 requires ALL students entering Kindergarten to have a full eye exam by a licensed physician or optometrist. The SNELLEN chart used in pediatrician offices for physicals is not sufficient for this requirement.

The Illinois Dept. of Public Health is working with the State Board of Health to finalize the required form. In this interim, you can use this draft form provided by the Illinois Optometric Association for the required eye exam. Illinois Eye Exam Form

It is a school requirement that your child entering Kindergarten get an eye exam and you turn in the completed form by 10/15/08. Please contact your school health office if you have any questions or concerns.

 

HEALTHFUL TIPS

Guidelines to consider when sending your child to school :

* Students should be free of fever (without use of medication), diarrhea or vomiting for 24 hours before returning to school. A temperature of 100°F by mouth or 99°F under the arm is considered a fever.

* A doctor’s note clearing return to school is required for students with unexplained rashes.

*Please keep your child home if cold symptoms or low energy level will interfere with your child’s ability to keep up with the school day activities.

 
Quick Facts That May Be Helpful to Know

* Physicals are required for all students entering ECK/Kindergarten, Sixth grade, and for students entering school for the first time. Physicals must be completed no earlier than 1 year prior to the student’s first day of school. Students transferring from an Illinois school may submit the physical on file from the previous school. Out of state/country transfer students must present a health examination that was completed within one year prior to entry into an Illinois school on a form comparable to the Illinois physical form. * Immunizations are required for school attendance. Refer to the web site listed below for required immunizations. Medical and Religious objections to any immunizations must be submitted in writing and approved by a district committee. http://www.idph.state.il.us/about/shots.htm

* Dental Examinations are required for students entering Kindergarten, Second grade, and Sixth grade. Dental Form

* Medication: In order for your child to receive any medication at school, you and your child’s doctor must complete the district’s medication order form. Administration of Medication Form

* Asthma: The incidence of children in the school setting with asthma has been increasing at an alarming rate. A medication permission form is required for the administration of any medication. Asthma Action Plan

* Food/Peanut Allergies: Please notify the nurse’s office if your child has any type of severe allergy that may require emergency attention. Forms for food allergies can be obtained through the following web site.http://www.foodallergy.org/

* Please call the school or nurse’s office if your child has or is being treated for a childhood or communicable disease such as: strep throat; chicken pox; rotavirus; hand, foot and mouth disease; fifth’s disease.

* Vision & Hearing Screenings are conducted by the DuPage County Health Department. All special education students, kindergartners, second graders, and students new to the district will be screened for vision and hearing. First and third graders will be screened for hearing. The staff nurses at Bower School will screen the fourth grade students for vision. If you have any concerns regarding your child’s vision or hearing, please contact one of the nurses.
